Use the application "Snag-It" and capture some example pages of the correctly completed pages of the amlm lite4 pages.
Put those up in your manual. It would eliminate all the issues of having to hang around waiting for you to get a chance to respond to our problems.
There are too many gaps in your video -- and it is too small. We cannot see the fields you are filling in, how, and which ones you are letting remain untouched.
I am considering upgrading to the 'pro' version, but this has not been an easy exercise. My Host server techs can only provide so much help. :(
